Everything you need to know before your first visit to our West Monroe office.

Welcome

Your first appointment is mostly about listening and learning. We want to understand your pain, your health history, and your goals, and give you time to ask questions. Here is how to make the visit as smooth as possible.

Scheduling your first visit

  1. Contact us. Call (318) 855-3291 or request an appointment online. If your doctor is referring you, they can fax records to (318) 737-7039.
  2. Check your insurance. We accept most major insurance plans; please call to verify your coverage. Some plans require a referral from your primary care provider.
  3. Complete your paperwork. Fill out new patient forms ahead of time if you can (see below).
  4. Come to your visit. Plan to arrive a little early to check in.

What to bring

  • A photo ID and your insurance card(s)
  • Your referral, if your insurance plan requires one
  • A list of all medications and supplements, with doses — including any blood thinners
  • A list of allergies
  • Reports or images from recent X-rays, MRIs, CT scans, or nerve studies, if you have them
  • Notes on treatments you’ve already tried (physical therapy, medications, injections, surgery) and how they worked
  • Your completed new patient forms

What to expect at your first appointment

  • A review of your medical history and how your pain affects daily life
  • A physical exam focused on the painful area and your nerves and muscles
  • A review of available imaging, and in some cases an order for new tests
  • A conversation about what may be causing your pain and the treatment options available, including their benefits and risks

If a procedure is recommended, it is usually scheduled for a separate visit, and we will give you instructions on how to prepare.

Insurance & payment

We accept most major insurance plans; call to verify. Coverage for specific procedures varies by plan, and our staff can help check before treatment. We accept cash, checks, and credit cards.

Appointment changes

If you need to cancel or will be running late, please call us as soon as possible so we can offer the time to another patient.
